• Personal Health Information entered is encrypted during transmission.

• Take your time to fully complete all sections on the application.

• Enter your Insurance Member ID and Group number.​

• 24-48 hours after submission, you will receive an email with the remainder of instructions (check your junk folder) before calling.​​​​​​

• The initial Psychiatric Evaluation is scheduled for 90-minutes.  ​​
